Short stay (one-day) surgery patients must pre-register three days (72 hours) prior to their operation so that
pre-operative tests may be completed. Waiting until the day of your surgery to register may result in postponement of cancellation of your operation.
Save yourself duplicate trips to the hospital by coordinating your pre-registration and lab tests with any
scheduled appointments you have at Santa Marta, such as X-rays or other tests. Check this brochure or
with your physician's office to determine which tests require appointments and what type of fasting is
needed beforehand.
Minors (under age 18) coming to the hospital for treatment must be accompanied by a parent or legal
guardian (who can present guardian papers). An adult relative or family friend is not sufficient to
authorize a minor's medical care.
Always bring California ID, insurance or HMO cards, current Medi-Cal and/or Medicare cards when-
ever you come to the hospital, as well as your physician order forms for tests.
Bring the medication you are currently taking.
Patients arriving for outpatient tests, pre-registration, short stay surgery or hospital admission, should
always stop at the information desk in the main lobby. You will then be called to admitting as soon as
a clerk is available. (If you have to wait a few minutes longer than someone who arrived after you, be
advised that surgery patients are usually given priority.)
When you register for hospital services, an admitting clerk will inform you of your right to make decisions
about medical treatment. Federal and state laws require that you be informed of your rights and given an
opportunity to complete an advance patient directive. A Durable Power of Attorney for Health Care form,
can be completed at registration which designates someone to make decisions for you, should you become
unable to do so. You are not required to complete this form, and we will be happy to answer any questions
you have.

Los pacientes de cirugia de estancia corta (de un dia), deben inscribirse tres dias (72 horas) antes de su
operacion, para que se les puedan hacer los examenes anteriores a la cirugia. El esperar hasta el dia de su
cirugia para inscribirse, puede ocasionar el que se atrase or se cancele su operacion.
Si llama al Hospital Santa Marta antes de programar su inscripcion, se le facilitaran los horarios que usted
necesita para los diferentes examenes con las instrucciones adecuadas.
Los menores (de 18aņos) que vengan al hospital para recibir tratamiento, deben ser acompaņados por uno
de sus padres o del tutor legal (quien pueda presentar los papeles de tutela). No es suficiente que un familiar adulto o un amigo de la familia autorice el cuidado medico del menor.
Siempre que venga al hospital, traiga su tarjeta de identificacion del Estado de California, las tarjetas del
seguro del HMO (Organizacion Para El Mantenimiento De La Salud), Las tarjetas actuales del Medi-Cal
Y del Medicare, ademas de los formularios con las ordenes de su medico para los examenes.
Traiga las medicinas que usted este tomando.
Los pacientes que llegan al hospital para recibir examenes como pacientes externos, cirugia de estancia
corta o para ser admitidos al hospital, deben ir siempre al mostrador de informacion en la sala de espera. Se le llamara a admisiones tan pronto como el empleado(a) pueda atenderle. (Si usted tiene que esperar un tiempo mas que alguien que llego despues de usted, se le informara que se le da prioridad a los pacientes de cirugia),
un empleado(a) de admisiones le informara de su derecho a tomar decisiones con respecto a su tratamiento
medico, cuando se inscriba para recibir los servicios de hospital. Las leyes Federales y Estateles requieren que le informemos acerca de sus derechos y le demos la oportunidad de llenar un formulario de orientacion para el paciente. Usted tambien puede llenar una carta poder (o consentimiento) para que tome decisiones
por usted en caso de que llegara a no tener la capacidad para tomarlas. Usted no tiene que llenar este formulario, y contestaremos con gusto cualquier pregunta que usted tenga.
